Case Management & Aftercare
While your child is in our care, office staff communicate regularly with field guides
to monitor the needs of your child. Case managers, clinicians and other qualified
staff review information regarding behaviors and progress towards treatment goals
in order to determine the best plan of care for your child. In addition, individual
and group updates are generated from the field and communicated to each family on
a weekly basis.
We see our therapeutic program as an important link in the ongoing continuum of
care, and so provide aftercare planning as an integral part of your child's long-term
success. We are dedicated to seeing that families and participants are able to access
the help that is available to them in their home communities. Always, our goal is
to work toward participants returning to their home and family. In order to reach
this goal, case managers work with the families and professionals involved with
each child to plan ongoing services and activities to support the child's success
after program.
